"Health Access Card"

"Health Access Cards" are provided by the FamilyPACT program, a state clinical program administered by the California Department of Public Health's Office of Family Planning. 

Neither Health Access Foundation nor Health Access California is affiliated in any way with the "Health Access Card" or the FamilyPACT program.
